Various prizes (valued from $25 to $1,000) will be randomly distributed around Tucson throughout the summer (end of May through August) and to individuals completing this survey. Restrictions do apply. Only one entry per household. Must be 18 years or older to enter. No purchase is necessary to enter. Winners may be notified in person, via e-mail, via phone, or prizes may be mailed directly to address given on form above. Odds of winning are based on the number of entries.
